Health Leads partners with communities and health systems to address systemic causes of inequity and disease by removing barriers that keep people from identifying, accessing, and choosing the resources everyone needs to be healthy. The organization received a Partnership Fund grant to shift its focus from patients to partnering with community-based organizations, health systems, and communities.
